Transitional Care Unit New York - Short Term Rehabilitation TCU

A PERSONAL APPROACH TO SHORT TERM REHAB

The Transitional Care Unit (TCU) in New York offers an expanded level of service to patients in need of short-term rehabilitation care, not exceeding 21 days, before returning home. It is an ideal option for individuals who can benefit from intensive rehabilitation, with the goal of transitioning from hospital to home at an improved level of functioning and mobility.

Our philosophy is to let the patient be our guide, helping our skilled specialists design an individualized treatment plan to meet the patient’s specific needs and goals. Each patient is cared for by an interdisciplinary treatment team under the direction of physicians specializing in rehabilitation medicine and orthopedic rehabilitation. Working collaboratively towards the patient’s goals, the team consists of rehabilitation nurses, physical and occupational therapists, case managers and other professionals. Family members are encouraged to participate in the rehabilitation process.

TRANSITIONAL CARE UNIT ADMISSIONS

Rehabilitation in the Transitional Care Unit is well suited for individuals recovering from hip or knee replacement surgery, fractures or other orthopedic conditions, or those medically deconditioned due to surgery, complications or other disorders.

To qualify for admission to the TCU, a patient must be admitted directly from an acute care hospital, where they have had a minimum three night stay. The patient must be able to participate in daily intensive therapy.
