New York State Department of Health’s Helen Hayes Hospital Selected for 2025-2026 Montefiore Health System Skilled Nursing Facility Collaborative

New York State Department of Health’s Helen Hayes Hospital’s Skilled Nursing Facility has been selected for a second time by the Montefiore Health System as a preferred provider of subacute rehabilitation services through Montefiore Health System’s Skilled Nursing Facility (SNF) Collaborative. This designation recognizes the Hospital’s long history of excellence in post-acute physical rehabilitation services for patients recovering from joint replacement surgery, stroke, heart attack, fractures, orthopedic conditions, chronic illness, medical deconditioning and other disabling conditions. This recognition, awarded for 2025-2026, marks the second two-year period that the Hospital’s SNF will be included in Montefiore’s Skilled Nursing Facility Collaborative.

Facilities chosen were selected by a team of Montefiore experts based on the facilities’ safety records, ability to care for patients with complex conditions, patient satisfaction scores and quality of care and services. Partner SNFs provide the highest possible quality of care and work with Montefiore Health System caregivers to ensure a seamless transition from acute to subacute care.
The Skilled Nursing Facility at Helen Hayes Hospital offers an expanded service to patients who need post-acute rehabilitation care before returning home. The flexible program serves patients at various stages of the rehabilitation process, making it an ideal option for individuals beginning or continuing their rehabilitation treatment. Each patient receives an individualized treatment plan crafted by an interdisciplinary team of experts to meet their needs and goals before returning home, including walking, climbing stairs, managing daily routines, transferring, and gaining strength. The program is Five-Star Rated by the Centers for Medicare and Medicaid Services, recognizing the Skilled Nursing Facility’s top scores in quality of patient care, staff expertise and excellence in rehabilitation outcomes.
